Long Term Condition Management
At Newham, in east London, we are delivering a Long Term Conditions Management programme which will ensure a more effective use of local resources. Working in league with all local stakeholders from across the health and social care economy, this programme includes eSAP assessment tools, electronic patient records and shared careplans to facilitate coordinated care. Serco also contributed our own Community Matrons to ensure the service got off the ground quickly.
