Care coordination

One of our specialties is the delivery of Care Coordination, in other words, smoothing and expediting the patient's journey through the various systems that can exist not only within a single hospital or multi-site Acute Trust, but between the Acute, Community and Primary Care systems.

Out of Hospital

We specialise in the integration of social, mental and community health care services in out-of-hospital settings. In our flagship Community Services contract with NHS Suffolk, for example, Serco coordinates care in community settings for a population of 500,000 people.

The Care Coordination Centre is the hub of our communication, integration and administration services. It provides a 24/7 Single Point of Access, can triage referrals, complete screening assessments, and coordinate care delivery. We deliver this in Suffolk Community Services contract and for Hertfordshire Adult Care Services.

Our model gives a Named Care Lead to referred patients, who will then create a personalised Care Plan in conjunction with the patient and family to 'wrap care around' the individual. We also create teams based around GP surgeries, including specialists, who will then care for patients in their local area, getting to know them well and increasing familiarity and trust.

In Hospital

Serco is currently developing a ground-breaking new tool for streamlining and improving the whole patient administration process in UK Acute hospitals. Following a pilot phase we have developed an IT enabled approach to improve customer contact services for patients, clinicians and staff.
